Patient Information

Your Office Visit

If your medical information is not already present in our office on the day of your appointment, please bring:

  • Insurance card
  • Photo identification
  • Completed registration forms (see forms section)
  • List of current medications
  • List of any food or drug allergies
  • List of questions you want to ask the doctors

Insurance

We accept Medicare and most health insurance plans. Many insurance policies require a referral from the patient's primary care physician before the patient can be seen by a specialist. Please be certain that we have proper authorization for evaluation by a Vascular Surgeon in order to avoid personal expense.

Generally, vein treatments are categorized as either Medically Necessary or Cosmetic. Insurance companies use varying criteria in order to determine if a vein treatment is to be considered "Medically Necessary." These criteria may include one or more of the following:

  • Lifestyle Disruption: The daily activities of the patient must be disrupted significantly.
  • Pain: The patient must be experiencing pain as a result of their vein disorder.
  • Failure of Conservative Measures: Other methods of treatment, such as compression hose, have failed to provide adequate relief.
  • Vein Size: Bulging veins larger than 4 mm are often considered medically significant.
  • Complications: Complications, such as phlebitis, bleeding veins, leg swelling and leg ulceration.

We will submit claims for "Medically Necessary" procedures and furnish you with all the financial information at our disposal so that you are able to make an informed decision concerning your vein treatment. Cosmetic procedures are not covered by heath insurance plans.

Payment Options

Co-pays and deductibles are due at the time of service. Payment methods available are: check, debit card, MasterCard, and Visa.

Cancellation Policy

We understand that your schedule may change and that emergencies arise. If you need to change or cancel your appointment, please call our office as soon as possible, (530) 345-9455. Please extend us this courtesy so that other patients may be seen.

Test Results

It is our plan to contact you regarding your test results. Please keep in mind that it may take a week for some test results to become available. Many times a follow-up appointment is scheduled so that together, you and your doctor can review the results and craft your treatment plan. If you do not hear from us, please contact our office at (530) 345-9455.

Hospitalization

If surgery is indicated, our office staff will coordinate a convenient date for you and the physician.
